What is an Affidavit of Change of Appearance?
This affidavit is used when your physical appearance has changed significantly and you need to formally confirm that you are the same person identified in an existing document — such as a passport, driver’s licence, or government ID. It is commonly required by financial institutions, government agencies, or courts when your current appearance no longer matches your identification.
When Would You Use This?
- Your passport or ID photo no longer resembles you due to significant weight change, injury, or surgery
- A financial institution is questioning your identity based on your photo ID
- You need to confirm your identity for a legal proceeding where your appearance has changed
- A government agency requires confirmation that you are the person shown in an older document
