What is a Proof of Life Affidavit?

When Would You Use This?

  • A foreign government or embassy requires proof of life to continue a benefit
  • An insurance company needs confirmation before releasing funds
  • An estate matter requires confirmation a person was alive on a specific date

How to Use This Template

Once complete, print and sign it in the presence of a notary public — do not sign beforehand. A commissioner of oaths may be used instead of a notary public for some documents, but not all. A notary public can sign all documents.
