What is a Proof of Life Affidavit?
A proof of life affidavit is a sworn statement confirming that a named individual is alive as of a specific date. It is commonly required by pension administrators, insurance companies, foreign governments, or financial institutions to continue benefit payments or confirm eligibility.
When Would You Use This?
- A pension or annuity provider requires periodic confirmation that a beneficiary is still living
- A foreign government or embassy requires proof of life to continue a benefit
- An insurance company needs confirmation before releasing funds
- An estate matter requires confirmation a person was alive on a specific date
