What is a Same Person, Different Name Affidavit?

This affidavit is a sworn statement confirming that two or more names refer to the same individual — for example, a birth name and a married name, or a legal name and a name used on older documents. It is commonly used when there is a discrepancy between names on different identification documents, and an organization needs confirmation that the same person is involved.

When Would You Use This?

  • Your name changed after marriage or divorce and documents show different names
  • A financial institution or government agency requires proof that two names belong to the same person
  • An estate matter involves documents under different names for the same individual
  • Immigration or travel documents show a name variation that needs to be explained
