A statutory declaration of loss is a sworn statement confirming that an important document or item of value has been lost, stolen, or destroyed. It is used to initiate replacement procedures with government agencies, financial institutions, insurance companies, and other organizations that require formal confirmation before reissuing documents or releasing funds.

Typical uses

This declaration is commonly used to replace a lost passport (in conjunction with a police report), to report a lost share certificate or bond, to claim insurance for a lost or stolen item, or to replace a lost land title, vehicle ownership certificate, or professional designation certificate.

What it should include

The declaration should clearly identify the lost item (including any serial or reference numbers), describe the circumstances of the loss, confirm that reasonable efforts have been made to locate it, and state that the declarant has not transferred, sold, or encumbered the item. It must be witnessed by a Commissioner for Oaths or Notary Public.
